Reports to: Clinical Director
Hours: 37.5 hours per week
Shifts: Monday-Friday - Must be flexible and able to adapt to changing environments
Status: Permanent Full Time

Position Summary
The Reactivation Care Centre (RCC) is a short-term, post-acute care unit for ALC patients who no longer require the level of care offered in an acute hospital setting. The RCC provides the necessary supports to assist patients in their transition from being in hospital to renovated home, supportive housing, RH, limited LTC, waiting for Rehab or to another appropriate care environment.
Operations Management
- Responsible for the management, operation, and organizational effectiveness of the Buttonwood RCC Units
- Ensure program goals and objectives are aligned with the hospital’s strategic business objectives
- Facilitate the interprofessional team model, ensuring that the model of care delivery is consistent with UHN’s mission, vision, values and strategic plans.
- Facilitate the development of service delivery solutions by collaborating with physicians, other multi-disciplinary teams, and other units/departments to ensure continuity and optimal client care
People Management
- Recruit, interview, hire and provide department orientation. Make recommendations to the Clinical Director and Transitional Care Program Management Team for compensation adjustments both aligned with and extending beyond UHN’s policies and procedures for all hires and internal transfers
- Identify and determines skill requirements and skill mix of staff to ensure consistency with the goals of the hospital and the Buttonwood RCC; evaluating the skill mix on an on-going basis
- Mentor and coach staff, and if required, discipline and terminate staff. Identify employee issues and concerns while ensuring matters are resolved in a manner which is both fair and consistent with professional standards, legislation, and UHN policies and procedures; Recommends/conducts appropriate disciplinary action, including termination, as required
- Knowledgeable of UHN attendance and performance policies and procedures
- Completes and submits appropriate forms to UHN People & Culture Teams
Quality & Planning
- Support organizational strategies and initiatives (i.e. client satisfaction) and collaborates with Management Team to develop and implement quality improvement strategies/proc esses which are aligned with UHN patient care standards and which enhance the patient experience, operational effectiveness and program efficiency
- Develops and establishes long- and short-term goals for outpatient rehabilitation services delivery across the continuum that addresses system gaps and aligns with Ontario Health priorities
- Develops program plans in conjunction with the Buttonwood RCC staff members and UHN Management Team
Financial Management
- Develop capital and operating budgets for the utilization of resources, including staff, equipment, supplies and space with input from the Management Team
Corporate Management
- Contributes to corporate efforts and initiatives, as appropriate, to enhance UHN mission, values and goals, including operational effectiveness and client and staff satisfaction; ensures policy and operational issues are well communicated within Program
- Contributes to UHN mission of exemplary patient care through client satisfaction evaluation and trend analysis
Qualifications
- At minimum completion of Bachelors Degree in Regulated Health Profession or recognized equivalent required, with a Masters Degree preferred
- 4 - 6 years practical experience providing care in a hospital environment with increased leadership/management responsibilities
- Experience in a Reactivation Post-Acute Care environment
- Membership in good standing with a Regulated Health Professional College
- Has knowledge of collaborative networks, primary care, and integrated health systems
- Has knowledge of applicable legislation affecting human resources (i.e. Human Rights, Employment Standards Act, Occupational Health & Safety Act, etc.)
- Excellent recognizing, analyzing and resolving problems skills
- Excellent decision making skills with the ability to form/defend independent judgment and to use good judgment in assessing difficult situations
- Demonstrated commitment to continuing education
- Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinion, backgrounds and characteristics bring richness to the challenge or situation at hand
