Position Summary
As part of the Blood Disorders Program, the Clinical Coordinator in the Thrombosis & Hemostasis Clinic provides specialized outpatient care to patients with thromboembolic disorders and anticoagulation management needs. The coordinator practices in accordance with the College of Nurses of Ontario standards, the UHN Professional Practice Model for Nursing, and the mission and values of UHN. The team at the Thrombosis and Hemostasis Clinic includes excellent healthcare professionals including physicians, nurse practitioners, allied health, administrative team and volunteers.
The role combines direct patient care with care coordination, patient education, and interdisciplinary collaboration. The Clinical Coordinator will apply advanced knowledge of anticoagulation therapy, monitor treatment outcomes (including lab results such as INR), and support patients and families through individualized teaching and care planning. The primary role will include long term follow up and support for patients on warfarin therapy.
Duties
- Provide expert clinical assessment and ongoing management of patients receiving anticoagulation therapy.
- Monitor laboratory results, perform warfarin and DOAC dose adjustments per standardized protocols, and escalate clinical concerns appropriately.
- Collaborate with hematologists, pharmacists, and the interdisciplinary team to develop and implement individualized care plans.
- Perform accurate and timely documentation in Epic and other health information systems.
- Provide comprehensive patient and family education regarding diagnosis, treatment plan, side effects, and self-management strategies.
- Support orientation and mentorship of new staff, sharing clinical expertise and promoting skill development.
- Participate in quality improvement initiatives, audits, and data collection to advance best practice and patient outcomes.
- Ensure compliance with UHN policies, accreditation standards, and infection control practices.
- Identify and respond to deteriorating patients, initiating emergency procedures as required.
Qualifications
- Current registration in good standing with the College of Nurses of Ontario (CNO).
- Completion of Basic Cardiac Life Support (BCLS), Level II Rescuer certification.
- B.Sc.N. preferred.
- Demonstrated clinical experience in medical nursing; experience in hematology, anticoagulation therapy, or thromboembolic disorders strongly preferred.
- Strong patient teaching abilities and effective communication skills.
- Excellent organizational skills with the ability to prioritize in a busy ambulatory setting.
- Ability to function collaboratively within a multi-disciplinary team and work independently when required.
- Strong clinical judgment, critical thinking, and decision-making skills.
- Proficiency with electronic health records (Epic preferred) and familiarity with provincial systems (Connecting Ontario, RM&R).
