Reports to: Clinic Manager, Bathurst
Hours: 37.5 hours per week
Salary: $39.07 - $56.00 per hour. (start rate non-negotiable and determined in accordance with collective agreement)
Shifts: Mondays - Saturdays (varied shifts: 08:30-16:30, 09:00-17:00, 12:00-20:00)
Status: Temporary Full Time (until aprox July 2, 2027)

Position Summary
The Toronto Western Family Health Team delivers primary care services through a multi-disciplinary, inter-professional approach to diverse patient populations. The Registered Nurse (RN) will be an integral member of both the Family Health Team and the University Health Network Nursing group. In this role, the RN supports patients across all stages of life, through varying states of wellness and illness, while collaborating with patients, families and other care providers. Responsibilities include holistic assessment, health screening, chronic disease management, health promotion, and patient education, with the goal of improving health outcomes and enhancing access to care.

Duties
- Provide complete comprehensive nursing assessments, with an understanding of the social determinants of health
- Complete triage of patients both in person and on the phone to aid in appropriate treatment and care decisions in relation to same day appointments, unscheduled care, and telephone calls from patients
- Assist in the functioning of the clinic to ensure smooth and efficient functioning
- Provide general primary care nursing to patients in keeping with nursing standards
- Develop and implement treatment plans/therapeutic interventions in consultation with patients and other interdisciplinary team members
- Provides health teaching and education, counseling, and health promotion and prevention of illness to patients
- Make appropriate hospital and community referrals and follow up as needed
- Assists in the functioning of the clinic to ensure smooth and efficient operation
Work collaboratively with other TWH UHN FHT staff and community organizations to improve client care, service delivery, and coordination - Preceptorship of Primary Care nursing students
Qualifications
- Completion of Bachelor Science in Nursing Degree required.
- Current certificate of registration from the College of Nurses of Ontario required.
- Basic Cardiac Life Support (BCLS) certification required.
- 3 to 5 years of demonstrated nursing experience working with patients and families in a multi-disciplinary and inter-professional team within a primary care setting required.
- Demonstrated experience working with a Family Health Team’s targeted patient populations: patients with chronic conditions particularly diabetes and vascular disease, new immigrants, young children and families, care for patients in the home, patients requiring access to mental health and/or addictions services required.
- Ability to effectively triage patients that walk in or call on the telephone required.
- Up to 6 months – 1 year of demonstrated experience using specialized medical platforms such as TELUS PSS Suite, CHIME, Ocean and EPIC required.
- Ability and willingness to complete home visits in conjunction with other care providers preferred.
- Knowledge of the broader determinants of health preferred.
- Knowledge of concepts of health promotion, disease prevention, behavior change. counseling, and individual and group counseling required.
- Knowledge and awareness of health equity and working with diverse communities required.
- Proven ability to problem solve effectively and independently, organizational skills (managing workflow, assessing and judging priorities, planning, multi-tasking).
- Experience in educating health professionals, including nursing and medical students.
- Commitment to patient-centered care with the ability to adapt to a diverse patient population.
- Demonstrated ability to work effectively within an interdisciplinary team.
- Availability to work flexible hours including days, evenings, and Saturdays and at both Family Health Team sites as required.
- Excellent interpersonal and crisis intervention skills.
- Experience working in Emergency or ICU where triage skills are required is an asset.
- Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand.
- Ability to perform duties in a professional and courteous manner, producing high-quality work while meeting deadlines in accordance with UHN standards.
