Salary range: $78,954 - $118,424 per annum
Hours: 37.5 hours/week
Status: Permanent Full-Time

Position Summary

The UHN Patient Education Specialist (Post Acute) helps create a culture of health literacy and patient engagement in care at post-acute care sites and programs at UHN. The Specialist coaches/teaches clinicians and non-clinical patient care providers (e.g., physicians, nursing, Health Professions, administrative staff, etc.) in the use of patient engagement care practices and resources available at UHN.

The Specialist applies the standards and best practice guidelines developed by UHN Patient Education & Engagement and Patient and Family Learning Centres and by keeping current with published literature in patient education. The Post-Acute Specialist will act as a knowledge sharer and connector among post-acute care programs and with Patient Education and Engagement and the Patient and Family Learning Centres. The Specialist will also bring themes from post-acute back to PE&E and the PFLCs to enrich knowledge on patient experience and other needs across the health system.

Travel between sites will be required. UHN post-acute sites currently include (but may expand in the future with UHN growth):

  • West Park Healthcare Centre
  • Toronto Rehab (Bickle, Lyndhurst, Rumsey, UC)
  • Hillcrest Reactivation Centre
  • Queen's Estate
  • Buttonwood Reactivation Care Centre

Duties

  • Deliver effective training and educational support for topics related to patient education and engagement care practices, primarily at UHN post-acute care sites
  • Liaise and connect with post-acute care sites and programs to build relationships, understand needs and strengths
  • Support post-acute members of TeamUHN to learn and use the UHN tools available to teach patients effectively
  • Coach clinicians to use and develop point of care patient education
  • Support Patient Education and Engagement planning and implementation activities
  • Facilitate patient- and caregiver-facing events and education in partnership with the PFLCs and post-acute partners
  • Assist the managers of Patient Education and Engagement and the Patient and Family Learning Centres to build post-acute needs into overall program strategies and operations
  • Help conduct work related to Quality, Evaluation and Research

Qualifications

  • At minimum 4 years practical and related experience and 2 years on the job training
  • Previous on-the-job computer/word processing, database and spreadsheet software experience
  • Experience in a Microsoft Office environment, including demonstrated expertise with Teams, Sharepoint, Excel, PowerPoint and Word
  • Strong customer service skills
  • Experience facilitating group education in virtual and in-person environments
  • Experience with plain language review and editing
  • Experience working with Patient Partners or people with lived experience
  • Experience working in a health care environment preferred
  • Previous experience working in a post-acute health care environment an asset
  • Previous experience with Quality Assurance an asset
  • Ability to perform duties in a professional and courteous manner and produce a high quality of work while meeting deadlines in accordance with UHN standards
  • Ability to liaise between hospital sites, multiple stakeholders and the Patient Education team members
  • Exercise initiative and good judgement with ability to multi-task
  • Compliance with confidentiality requirements
  • Excellent organizational, interpersonal, and communication skills
  • Client service oriented, with the ability to effectively work with diverse populations
  • Excellent verbal and written skills
  • A second language an asset
