As an integral member of the IMPACT team, the Peer Support Worker, works collaboratively with members of the multi-disciplinary team to provide a service to patients with severe mental health challenges. The principal responsibilities include: providing support to IMPACT clients in meeting their treatment and rehabilitation goals; facilitating client social and recreational opportunities; providing a valuable perspective for service users within the Assertive Community Treatment (ACT) model and the Mental Health System; participating in the training and supervision of students; performing cross-functional and/or other duties as assigned or requested to contribute to the efficient and effective provision of service.
- Provide supportive, client-centered care within the ACT model, including visiting clients in the community, helping maintain a safe environment and connecting clients with community resources and services.
- Advocate for clients to help them access the supports they need, and provide education to clients, and families about their rights and how to access and maintain services.
- Helps connect clients with social and recreational opportunities that match their interests and needs, including accompanying them to appointments, activities, and community events when appropriate.
- Supports and encourages client participation in social and recreational activities by helping plan, coordinate, and explore different opportunities, and by sharing information about available events and programs
- Draws upon lived experience to provide invaluable knowledge to staff, students and the community at large, including imparting knowledge of different/appropriate types of techniques, i.e. listening, questioning, etc.
Qualifications
- At minimum, a completion of a high school diploma
- Completion of recognized Human Services or Social Services diploma is an asset
- Two years work experience in Mental Health and/or Substance Use Preferred
- Lived experience with Mental Health challenges and/or Substance Use
- Peer Support Certificate or training is an asset
