Salary Range: $53.09 to $66.36 per hour
Hours: 37.5 hours per week
Shifts: Monday to Friday, mostly Days with some Evenings
Status: Temporary Full Time (18 month contract)

Position Summary

The NURSE PRACTITIONER demonstrates autonomy while assuming and having sole accountability and the authority to independently communicate a diagnosis, prescribe drugs and order a range of diagnostic tests, the principal responsibilities include: utilizing and demonstrating advanced knowledge and decision making skills to conduct comprehensive assess, diagnose and manage the care of patients; planning and implementing strategies to promote health and prevent illness with individuals, families and groups; delivering, coordinating and participating in educational and professional development activities/programs.

The NURSE PRACTITIONER will also provide coverage (as required) for UHN’s Virtual Emergency Department, providing care to patients through video visits for urgent, non-life threatening issues via same day online bookings.


The Nurse Practitioner (NP) serves as the Most Responsible Provider (MRPr) for a diverse panel of patients across the lifespan, delivering comprehensive primary care within the Toronto Western Family Health Team. In this role, the NP conducts advanced health assessments, diagnoses and manages acute and chronic conditions, and provides evidence‑informed preventative care. The NP works collaboratively within an interdisciplinary team—including physicians, nurses, social workers, pharmacists, and other interdisciplinary health professionals—to coordinate holistic, patient‑centred care. With a focus on accessibility, continuity, and quality improvement, the NP supports patients through health promotion, chronic disease management, and navigation of community and hospital-based services.

Duties

  • Provide full scope comprehensive primary care to individuals across the lifespan as MRPr (Most Responsible Provider)
  • Support and engage in team research, and education
  • Work in collaborative practice within a health care team, along with independent practice within full-scope of a RN (EC)
  • Demonstrate leadership as a change agent and support advancement of nursing practice within the FHT by supporting, coaching, and training FHT RN's.

Qualifications

  • Completion of a recognized Masters Degree program in Nursing
  • Primary Care Nurse Practitioner Certificate, or in progress
  • Current certificate of (licensure)/registration with the College of Nurses of Ontario as RN (EC) or in process of obtaining
  • Current certification in Basic Cardiac Life Support
  • Five (5) years of nursing experience, including one (1) year in an advanced role. MRPr (Most Responsible Provider) experience preferred
  • Ability and commitment to develop a collaborative practice within an interprofessional team
  • Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards
  • Exercise initiative and good judgment with ability to multi-task
  • Compliance with confidentiality requirements
  • Effective interpersonal and communication skills
  • Knowledge of applicable legislative, UHN and/or departmental policies
  • Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand
  • Knowledge of quality improvement
  • Excellent organization and time management skills
  • Program planning and evaluation methodology skills
  • Ability to utilize information technology
